Se caracteriza habitualmente por fiebre, escalofríos, vómitos, anemia, hepatoesplenomegalia, leucocitopenia, hipergammaglobulinemia, emaciación, y piel de color gris terráceo. La enfermedad se clasifica en tres tipos principales según la distribución geográfica: india, mediterránea (o infantil) y africana.
